Ministry Events
As good stewards of our time, talents, and facilities we ask that anyone wanting to host an event at Belmont Assembly of God submit a Project Map and that it be submitted AT LEAST 2 months prior to the event.
All Ministry Event Project Maps will be submitted and approved by Pastor Joey and Staff.
Making A Vow
A baby dedication is a ceremony in which believing parents, and sometimes entire families, make a commitment before the Lord to raise that child according to God's Word and God's ways.
Typically, the ceremony involves the reading of key Bible passages and a verbal exchange in which the minister asks the parents (and godparents, if so included) if they agree to raise the child according to several criteria.

Online Wedding Request Form
Weddings may be held on Friday, Saturday, or Sunday. All requests must be completed at least 2 months in advance. We highly recommend that you do not book your reception hall until you have confirmed your date with the church. Your wedding date will not be confirmed until after Belmont Assembly of God gives a final approval to proceed.
**Please note that Belmont Assembly of God will be undergoing renovations periodically throughout 2022 in order to update our facilities. The main sanctuaries should not be affected, however, other areas of the Church may have signs of construction.

Incident & Accident Reports
This form shall be completed by Belmont Assembly of God staff, elders/deacons or ministry leadership whenever anyone in attendance at or on the premise of Belmont Assembly of God is involved in an incident or accident which resulted in personal injury or property loss - EXCEPT for occupational (worker's compensation reportable) or automobile related accidents.
This form is an optional addition to the main incident and accident report form.
It would typically be used to help explain how a personal injury or accident occurred when there are more complicated details to describe that cannot be fully addressed on the main form. it may also be used in the event of property damage or theft where similarly there are extensive details to document by a witness to the incident.
We recommend that this supplemental form be filled out by the same witnesses identified on the main form and shall become a part of the primary form when submitted.
